Community Support
Community Support Services (CSS) are designed to assist, teach and support individuals who reside in their own environment, such as an apartment, house, or family home, and require minimal but on-going staff assistance. The most important component in the CSS program is the health and safety of the individuals we serve.
Any range of daily living supports an individual may require to maintain an independent lifestyle in the community can be provided in the CSS program. Each person served has unique abilities and needs, which are evaluated upon referral to develop a service plan. However, CSS participants must be able to demonstrate the ability to self-medicate (as applicable) and have the basic skills to function with some degree of success and safety in the community.
Staff persons are assigned according to the need level of the individual. An individual with several needs may have two staff persons assigned to address the issues outlined in the Individual Service Plan. All of the staff is trained to ensure the best quality of service and promote independence.
Examples of Services
- Assist with setting up medical or counseling appointments; ensure that appointments are kept through prompting and follow-up; if desired, accompany the individual on appointments; follow along to ensure medical orders are followed and medications are administered properly
- Teach money management (paying rent and bills on time, maintaining bank accounts, saving for special purchases, etc.)
- Reinforce the procedures necessary to remain safe and healthy in one’s own home
- Aid with grocery shopping, menu planning, and personal shopping for clothes and household items
- Provide appropriate training to use public transportation safely
